HIFU for a Double Chin in Tokyo: Fat, Loose Skin and Treatment Planning

A double chin can be caused by excess fat beneath the chin, loose skin, or a combination of both. High-Intensity Focused Ultrasound (HIFU) may be considered for some of these concerns, but the cause of the fullness determines what treatment can realistically achieve.

At MATSURIKA CLINIC GINZA, we offer ULTRAcel Zi, a HIFU device with Dot and Linear modes designed for different treatment purposes. Before choosing a mode or treatment area, it is important to assess the tissue beneath the chin, the surrounding jawline, and your overall facial shape.

This guide explains what to discuss during a double-chin HIFU consultation in Tokyo, including treatment areas, available options, fees, limitations, and potential risks.

Is Your Double Chin Caused by Fat, Loose Skin or Both?

A double chin is not always caused by excess fat. Fullness beneath the chin can also result from loose skin, changes in the jawline, or the natural shape of the chin and neck.

The distinction matters because a procedure intended to tighten tissue is not necessarily the right treatment for someone whose primary concern is excess fat or facial structure.

What Your Doctor Checks Under the Chin

A physician should assess both the amount of tissue beneath the chin and the condition of the surrounding skin before recommending HIFU.

The examination may include:

  • Submental fat: Whether there is a noticeable fat deposit beneath the chin.
  • Skin laxity: Whether the skin has lost firmness or hangs loosely.
  • Jawline and chin shape: Whether facial proportions contribute to the appearance of a double chin.
  • Surrounding facial volume: Whether the cheeks or lower face are already relatively thin.
  • Previous treatments: Whether earlier HIFU, radiofrequency (RF), injections, or surgery may affect treatment planning.

Weight changes, medical history, and current medications may also be relevant.

At MATSURIKA CLINIC GINZA, our physicians assess your concerns and medical history before discussing treatment options. The goal is to identify which area needs attention rather than assuming that every double chin requires the same approach.

Why Appearance Alone Does Not Determine the Treatment

Two people may have similar-looking fullness beneath the chin but require different treatment plans.

For example, someone with a localized fat deposit and relatively firm skin may have different treatment needs from someone with little excess fat but noticeable loose skin.

A third person may have both concerns. Others may have a naturally less prominent chin, which can make the area beneath it appear fuller even without substantial excess fat.

HIFU cannot change the underlying bone structure or produce the same degree of fat removal or skin correction as surgery. Your physician should explain which part of your concern the proposed treatment is intended to address.

When to Discuss HIFU for a Double Chin

HIFU is worth discussing when fullness or skin laxity beneath the chin is a concern and you would prefer a procedure without surgical incisions.

However, skin tightening and fat reduction are different treatment goals. The proposed treatment should reflect which change matters most to you.

Discussing Skin Tightening and Fat-Related Concerns Separately

HIFU delivers focused ultrasound energy into selected tissue layers, producing controlled heating.

Depending on the device, treatment mode, and targeted tissue, this energy may be used to encourage tissue tightening or address localized fullness.

For a double chin, it helps to distinguish between two goals:

  • Skin tightening: Improving the appearance of loose or less firm tissue beneath the chin.
  • Fat-related contouring: Addressing localized fat that contributes to fullness beneath the chin.

These goals may overlap, but one treatment does not necessarily address both to the same degree.

At MATSURIKA CLINIC GINZA, ULTRAcel Zi provides separate Dot and Linear modes. We can discuss these options in relation to your main concern, while considering whether HIFU is suitable at all.

Questions About the Device and Available Modes

ULTRAcel Zi uses HIFU energy and offers two different patterns of heat delivery.

ModeEnergy deliveryMain treatment purpose
DotFocused ultrasound energy delivered at discrete points into selected tissue layersTissue tightening and lifting-related concerns
LinearFocused ultrasound energy delivered in a linear heating pattern across the targeted areaFat-related contour concerns, including fullness beneath the chin

Dot treatment is intended to address tissue laxity, including the deeper supporting tissues and skin layers selected for treatment.

Linear treatment is designed to heat a broader area of targeted tissue and is offered for fat-related concerns around the cheeks and under the chin.

Linear mode does not guarantee a particular amount of fat reduction. The amount of tissue, its distribution, and the treatment settings affect what change is possible.

Ask which mode is proposed, which tissue the physician intends to treat, and what the realistic limitation is for your particular double chin.

Planning the Treatment Area Under the Chin

The area you want to improve is not always identical to the area that should receive treatment. The physician needs to assess the chin, jawline, and neck together before deciding where ultrasound energy should be delivered.

This is particularly important if you are concerned about unwanted changes in facial volume.

Under the Chin, Jawline and Neck: Which Areas Are Included?

The area directly beneath the chin is called the submental area. It is different from the jawline, cheeks, and neck.

These areas are connected visually, but they should not be treated as interchangeable when reviewing a quotation.

For example:

  • Under the chin: The main area of concern when submental fullness is present.
  • Jawline and lower face: Areas that may affect how defined the lower facial contour appears.
  • Neck: An additional area to assess when loose skin or neck folds contribute to the concern.

At MATSURIKA CLINIC GINZA, ULTRAcel Zi includes treatment plans for the face and neck, as well as a Linear plan covering the cheeks and under the chin.

Ask whether the quoted plan includes the specific area you want treated. A full-face package should not automatically be assumed to include every part of the neck or a particular submental treatment pattern.

How Your Facial Shape Affects the Consultation

The treatment plan should also account for areas where you want to preserve facial volume.

For example, a person with fuller cheeks and noticeable submental fat may have different priorities from someone with naturally hollow cheeks who mainly wants the loose skin beneath the chin assessed.

Unwanted facial volume loss is a concern worth discussing before energy-based treatment. Treating more areas or delivering more energy does not automatically produce a more suitable result.

Tell your physician if you are concerned about cheek hollowing, an overly sharp jawline, facial asymmetry, or changes from previous cosmetic procedures.

These concerns help determine which areas should be considered, which should be avoided, and whether another approach would be more appropriate.

ULTRAcel Zi Modes, Treatment Areas and Fees

MATSURIKA CLINIC GINZA offers ULTRAcel Zi Dot and Linear treatments with different treatment areas and shot counts. For a double-chin consultation, the important question is not simply the price per session, but which treatment objective and anatomical area the plan covers.

ULTRAcel Zi Treatment Plans and Prices

The following are the standard listed fees for ULTRAcel Zi.

ModeTreatment areaShot countSessionsFee
DotFull face, including around the eyes500 shots1JPY 100,000
DotFull face, including the eye area, plus neckListed as 700 neck shots1JPY 133,000
LinearCheeks + under the chin300 shots1JPY 72,000
LinearCheeks + under the chin300-shot plan3-session packageJPY 205,200

All amounts include Japanese consumption tax. The 3-session Linear package should not be interpreted as guaranteeing a particular outcome.

The treatment page also identifies the under-chin area among the concerns ULTRAcel Zi is intended to address. The exact allocation of shots within the selected treatment area is determined through treatment planning.

For double-chin treatment, confirm the proposed mode, specific treatment area, shot count, and person performing the procedure before accepting a quote.

Prices booked through affiliated partners or external services may differ from the standard fees.

ULTRAcel Zi is an elective cosmetic procedure and is not covered by Japanese public health insurance.

Consultation and examination without treatment cost JPY 11,000 (tax included). If you require language support, extended appointment time is allocated and some fees may differ. Please confirm the amount applicable to your booking.

What to Confirm About the Treatment and Device

ULTRAcel Zi is a medical HIFU system that is not approved in Japan under the Pharmaceuticals and Medical Devices Act. We obtain the device through an authorized domestic distributor, Jeisys Medical Japan, Inc.

The device has regulatory authorization from South Korea’s Ministry of Food and Drug Safety (MFDS). Authorization in South Korea does not constitute approval in Japan, and it does not guarantee a specific cosmetic outcome.

During your consultation, ask about the intended target tissue, expected degree of change, and limitations of the proposed mode.

The treatment should be explained in relation to the area being treated, rather than assuming that all HIFU devices or treatment settings have the same effects.

HIFU Limitations, Risks and Other Options

HIFU cannot reliably correct every cause of a double chin. Its usefulness depends on the amount of submental fat, skin laxity, facial anatomy, and the degree of change you hope to achieve.

Potential risks should also be considered before deciding on treatment.

When HIFU May Not Match Your Main Concern

Another approach may need to be considered when:

  • The main concern is a substantial amount of fat beneath the chin.
  • There is considerable excess skin that requires more correction than a nonsurgical treatment can reasonably provide.
  • The apparent double chin is mainly related to chin projection or skeletal structure.
  • The face already has limited soft-tissue volume, making further volume reduction undesirable.
  • Medical conditions, implants, skin inflammation, or recent cosmetic treatments make HIFU unsuitable.

Other treatment categories include fat-targeting injections, surgical fat removal, and procedures for skin laxity. They differ in invasiveness, recovery, benefits, and risks.

MATSURIKA CLINIC GINZA also offers other skin-tightening technologies, including Ultherapy PRIME (focused ultrasound), Thermage FLX (monopolar radiofrequency), and Sofwave (SUPERB ultrasound). These systems have different energy-delivery methods and treatment purposes; they are not interchangeable fat-removal treatments.

Ultherapy PRIME and Thermage FLX have U.S. FDA clearances for specified uses, but both devices are unapproved in Japan under the Pharmaceuticals and Medical Devices Act. U.S. FDA clearance does not mean approval in Japan. Sofwave is approved in Japan for improving wrinkles on the face and neck, not specifically for removing submental fat.

A consultation should clarify whether any of these options addresses your principal concern. We do not offer them as universally equivalent solutions for a double chin.

Discussing Pain, Recovery and Follow-Up

ULTRAcel Zi may cause heat-related discomfort, stinging, or an aching sensation during treatment.

Potential risks and side effects include redness, swelling, tenderness, bruising, burns, altered sensation such as numbness or tingling, nerve injury, and unwanted changes in facial volume. Some symptoms resolve within days or weeks, but the duration and severity cannot be predicted for every patient.

Tell your physician if you have an implanted medical device, metal or other foreign material near the treatment area, an active skin infection, significant skin irritation, relevant medical conditions, or a history of abnormal scarring. ULTRAcel Zi is not offered during pregnancy or breastfeeding.

Please also disclose your medications and recent treatments. At our clinic, treatment cannot be provided if you have undergone HIFU or RF treatment within the previous three months, received certain skin treatments at another medical institution within the previous month, or had significant sun exposure or sunburn within the previous month. Your physician must still assess your current condition.

After ULTRAcel Zi, gentle cleansing, makeup, and bathing are generally permitted, but avoid vigorous rubbing and follow the aftercare instructions provided for your treatment. Moisturizing and sun protection are important.

If you have questions or concerns after returning home, you can contact MATSURIKA CLINIC GINZA by email or LINE. For severe pain, sudden swelling, fever, or other potentially urgent symptoms, seek prompt medical care at a local medical facility.

Double Chin HIFU Consultation Checklist

Bring information that helps your physician understand both your desired facial contour and your medical history. This makes it easier to discuss treatment suitability, costs, and aftercare without relying only on a photo or advertised package.

What Should I Bring to My Consultation?

Before your appointment, prepare the following:

  1. Your main concern: Is it fat beneath the chin, loose skin, an undefined jawline, or a combination?
  2. Your treatment goals: Which changes are you hoping for, and which features, such as cheek volume, do you want to preserve?
  3. Previous treatment details: Include dates and areas treated with HIFU, RF, injections, thread lifts, or surgery, including procedures performed outside Japan.
  4. Medical information: Bring your medication list and disclose relevant conditions, allergies, implants, and recent sun exposure.
  5. Your schedule and questions: Explain any travel plans, ask about recovery, and clarify how to contact the clinic afterward.

English-speaking physicians and staff are available at MATSURIKA CLINIC GINZA. Translation tools can also help support communication.

If you require language assistance, use the dedicated booking option for non-Japanese speakers. Patients who do not speak English or Japanese are encouraged to arrange a professional medical interpreter and bring documentation of the interpreter’s qualifications.

How Should I Compare Quotes and Before-and-After Photos?

Compare HIFU quotations using the same information rather than comparing prices alone.

Check:

  • Device name and treatment mode
  • Specific areas included, especially under the chin and neck
  • Shot count and number of sessions
  • Whether the treatment is performed by a physician or another qualified clinician
  • Additional consultation or pain-management fees
  • Expected changes, limitations, potential risks, and follow-up arrangements

Before-and-after photos should also be reviewed carefully. Look for the treatment name, areas treated, number of sessions, timing of the after photo, cost, and risks.

Changes in posture, head position, lighting, or body weight can affect the appearance of a double chin in photographs. A result shown for another patient does not establish what your own treatment will achieve.

Frequently Asked Questions About Double-Chin HIFU

1. Can HIFU treat just the area under my chin?

The possibility of treating only the area beneath your chin depends on the available plan and your medical assessment. ULTRAcel Zi includes a Linear treatment option for fat-related concerns beneath the chin, but not every advertised package covers the same areas. Please confirm whether an under-chin-only plan is available for your booking, along with the shot count and fee.

2. Does HIFU remove double-chin fat permanently?

HIFU may produce changes in localized submental fat, but it cannot guarantee complete or permanent fat removal. The degree of change depends on the device, treatment settings, and your anatomy. Weight changes and aging may also affect your contour over time.

3. Can treating a double chin with HIFU make my cheeks look hollow?

Unwanted facial volume loss is a possible concern with energy-based treatments, especially when the cheeks already have limited fat. A treatment plan focused on the double chin should not automatically include every part of the cheeks. Tell your physician which areas you want to preserve and discuss the proposed treatment boundaries.

4. Is double-chin HIFU painful, and what side effects are possible?

HIFU can cause heat, stinging, or aching during treatment. Afterward, redness, swelling, tenderness, bruising, or changes in sensation can occur; burns and nerve injury are also potential risks. Your physician should explain the risks and pain-management arrangements relevant to your treatment.

5. How much does ULTRAcel Zi cost for a double chin in Tokyo?

At MATSURIKA CLINIC GINZA, the standard ULTRAcel Zi Linear plan covering the cheeks and under the chin is JPY 72,000 for 300 shots, tax included. A 3-session package costs JPY 205,200. The final quotation should confirm the treated area, applicable booking fees, and any additional costs, particularly when language support is required.

Summary: Choosing HIFU for a Double Chin Starts With Identifying the Cause

HIFU may be considered for selected double-chin concerns, but fat, loose skin, and facial structure require different treatment planning. Before deciding, clarify which tissue is contributing to the fullness beneath your chin, whether skin tightening or fat-related contouring is the main goal, and which areas should be treated or avoided.

MATSURIKA CLINIC GINZA offers ULTRAcel Zi Dot and Linear modes, along with other options for skin laxity. Our physicians can assess your concerns, medical history, and facial shape, then explain the available treatments, their limitations, fees, and potential risks.

If you are considering HIFU for a double chin in Tokyo, you can book a consultation to discuss an appropriate treatment plan before making your decision.
