Sagging Jawline Treatment in Tokyo: Choosing Options by Cause
A less-defined jawline does not always mean that your skin simply needs lifting. Loose skin, changes in facial fat, loss of volume, and changes in the underlying support structures can all affect the contours of your lower face.
The right treatment depends on what is causing the change, where it appears, and how much improvement you want. Ultrasound and radiofrequency treatments may help with skin firmness, while injectable treatments address different concerns. More pronounced sagging may require a surgical consultation.
At MATSURIKA CLINIC GINZA, we assess the face as a whole rather than choosing a treatment based only on the appearance of the jawline. This guide explains what to look for, which treatments to discuss, and how to compare your options when seeking jawline treatment in Tokyo.
Why Your Jawline May Look Less Defined
Your jawline can lose definition for several reasons, and more than one may be present. Identifying the main contributing factor helps determine whether skin tightening, volume correction, fat reduction, or another approach deserves consideration.
Skin Laxity, Fat, and Volume Changes
A softer jawline may reflect changes in the skin, the underlying fat compartments, or the shape and support of the face.
| What you notice | Possible contributing factor | What to discuss |
| Loose skin along the lower cheeks or jaw | Reduced skin elasticity and changes in supporting tissues | Whether a skin-tightening or lifting treatment could help |
| Fullness beneath the chin | Localized submental fat, loose skin, or both | Whether the main concern is fat, skin laxity, or a combination |
| Jowls near the corners of the mouth | Changes in cheek fat position, skin laxity, and facial support | Whether tightening alone can address the desired change |
| Hollow cheeks or a less-supported lower face | Loss or redistribution of facial volume | Whether volume restoration would be more relevant than further tightening |
These patterns are not diagnoses. For example, a double chin may involve both fat and loose skin, while visible jowls may become more prominent as the cheeks lose volume.
The chin and jawbone also contribute to facial shape. A skin-tightening device cannot change their underlying structure.
What a Consultation Can Help Clarify
A consultation can help distinguish the visible problem from its underlying causes. Your physician should assess more than the area that looks loose in a photograph.
At MATSURIKA CLINIC GINZA, our approach includes reviewing the pattern of sagging across the face, particularly the cheeks, area around the mouth, jawline, and beneath the chin. We use this assessment to discuss treatment areas and the changes each procedure may realistically address.
Important points to review include:
- Location: Is the main concern the lower cheek, the edge of the jaw, beneath the chin, or the neck?
- Tissue characteristics: Is the area mainly loose skin, localized fullness, volume loss, or a combination?
- Treatment history: Have you previously received high-intensity focused ultrasound (HIFU), radiofrequency (RF), fillers, thread lifts, or other facial procedures?
- Personal goals: Are you looking for improved skin firmness, less under-chin fullness, or a more substantial change in facial contour?
Your physician will also review your medical history, medications, skin condition, and factors that could make a procedure unsuitable.
Matching Treatment Options to Your Main Concern
Jawline treatments work on different tissues and do not produce the same type of change. Skin-tightening devices use energy to heat selected tissues, while injections may restore volume or address localized fat.
Discussing Skin Tightening and Lifting Treatments
At MATSURIKA CLINIC GINZA, we offer several energy-based treatments for facial sagging, including Ultherapy PRIME, Thermage FLX, and Sofwave.
| Treatment | Energy source and main target | Treatment purpose and limits |
| Ultherapy PRIME | Focused ultrasound (HIFU); selected depths within the skin and deeper supporting tissues | Addresses tissue laxity and lifting concerns. It does not restore lost facial volume or surgically remove excess skin. |
| Thermage FLX | Monopolar radiofrequency (RF); heating across the dermis and underlying tissues | Used for skin firmness and tightening. It does not replace treatments for facial volume loss or substantial excess skin. |
| Sofwave | SUPERB™ (Synchronous Ultrasound Parallel Beam) ultrasound; primarily the dermis, around 1.5 mm | Uses dermal heating to address wrinkles and skin firmness. It does not restore lost volume or surgically reposition deeper facial tissues. |
Ultherapy PRIME uses DeepSEE real-time ultrasound imaging to visualize tissue during treatment. Our physicians consider the treatment area, target depth, and number and distribution of shots rather than relying on shot count alone. Ultherapy PRIME is performed by a physician.
Thermage FLX uses radiofrequency energy rather than ultrasound. We offer both physician-performed and nurse-performed treatment plans, with different fees.
Sofwave uses a different ultrasound delivery pattern from HIFU and primarily targets the dermis. At our clinic, physicians with specialized training perform Sofwave treatments and adjust the treatment area and settings to the patient’s skin condition and concerns.
No single device addresses every cause of jawline sagging. We also offer Ultherapy and ULTRAcel Zi. You can explore these options in our treatment menu.
Regulatory information: Ultherapy PRIME and Thermage FLX are FDA-cleared medical devices in the United States for their respective cleared uses. FDA clearance does not mean that every jawline treatment purpose has been cleared, nor does it constitute approval in Japan.
The Ultherapy PRIME and Thermage FLX devices used at our clinic have not been approved in Japan under the Pharmaceuticals and Medical Devices Act. Ultherapy PRIME is obtained through an authorized domestic distributor, Cynosure Japan / Syneron Candela K.K., while Thermage FLX is obtained through JMEC.
Sofwave has Japanese regulatory approval for improving moderate or severe wrinkles of the face and neck through ultrasound heating of the dermis. That approval does not mean that every form of facial lifting is an approved use, or that results are guaranteed.
When Other Treatment Approaches May Be Considered
Skin tightening may not address your main concern if the jawline looks less defined because of localized fat or loss of facial volume.
When facial volume has decreased: Hyaluronic acid fillers may restore volume in selected facial areas. At MATSURIKA CLINIC GINZA, our physicians assess the treatment location, product, and amount needed. Fillers do not remove excess skin, and adding volume is not appropriate for every type of jowl.
When fullness beneath the chin is the main concern: Treatment targeting localized fat may be discussed. Our clinic offers FatX Core fat-dissolving injections for the under-chin area. These injections address fat rather than lifting loose skin. FatX Core has not been approved in Japan under the Pharmaceuticals and Medical Devices Act. Our clinic obtains it through a domestic agency using physician-directed import procedures. Approval of a different deoxycholic-acid product in another country does not establish approval of FatX Core.
When significant tissue descent or excess skin is present: Non-surgical tightening may not provide the amount of repositioning you want. A consultation with a qualified surgeon about procedures such as a facelift or neck lift may be more relevant. These surgeries involve their own risks, scars, and recovery requirements and should not be treated as interchangeable with energy-based procedures.
The key question is which tissue needs to change to achieve your goal, and whether the proposed treatment can reasonably produce that change.
How to Compare Jawline Treatment Plans in Tokyo
Compare treatment plans by the area treated, who performs the procedure, the number of shots or amount of product, and the full cost. Two plans described simply as “jawline treatment” may include different services.
Treatment Area, Provider, and Total Cost
Below are examples of our treatment fees. All prices are in Japanese yen (JPY) and include Japanese consumption tax.
| Treatment | Treatment area or quantity | Provider | Price (JPY, tax included) |
| Ultherapy PRIME | Full face, including under the chin; 348 shots | Physician | ¥277,820 |
| Thermage FLX | Full face and under the chin; 300 shots | Physician (certified doctor) | ¥327,800 |
| Thermage FLX | Full face and under the chin; 300 shots | Nurse | ¥272,800 |
| Sofwave | Full face and under the chin | Physician | ¥308,000 |
| Hyaluronic acid filler (selected Allergan products) | One syringe of Voluma, Volift, Volbella, or Volite | Confirm the injection provider when booking | ¥85,000 |
| FatX Core | 1 cc; under-chin treatment available | Confirm the injection provider when booking | ¥15,000 |
These are price examples, not equivalent treatment plans. A full-face device treatment cannot be compared directly with one syringe or 1 cc of an injectable product. The amount required, treatment coverage, number of sessions, and total cost depend on the assessment and agreed treatment plan.
We also charge the following fees when applicable:
- Consultation and examination only: ¥11,000 (tax included) when no treatment is purchased.
- Topical anesthetic cream: ¥3,300 (tax included).
- Nerve block anesthesia: ¥5,000 (tax included).
Anesthesia charges apply when the relevant option is used and billed separately. Some bookings through partner services have different pricing. Patients requiring language support should confirm any applicable fee differences before booking.
These cosmetic treatments are self-pay and are not covered by Japanese public health insurance. Review our English price list and confirm the full estimate before agreeing to treatment.
Reading Before-and-After Photos in Context
Before-and-after photos can illustrate the changes recorded for an individual patient, but they cannot predict your results.
When reviewing jawline treatment photos, check:
- The exact treatment, area, number of shots or amount injected, and number of sessions.
- The interval between treatment and the follow-up photograph.
- Whether the images use comparable angles, lighting, facial expressions, and head positions.
- The stated cost and potential risks or side effects.
- Whether other treatments contributed to the photographed result.
You can review our Sofwave cases, treatment areas, and follow-up information on the Sofwave treatment page. Some photographs were taken three months after treatment, while another was taken immediately after treatment. These are different observation periods and should not be compared as if they showed the same stage of results.
Fees shown alongside older cases may differ from our current prices. Individual results vary, and photographs do not guarantee a comparable outcome.
Expected Changes, Risks, and Treatment Limitations
Jawline treatment may improve skin firmness or address a specific contour concern, but it cannot stop facial aging or correct every structural change. Potential benefits need to be weighed against the treatment’s limits, discomfort, complications, and follow-up needs.
Setting Goals With Your Doctor
Before deciding on treatment, describe the change you want in concrete terms. You might want less fullness beneath the chin, improved firmness around the lower cheeks, or a less visible fold near the jaw.
Your physician can then explain whether your goal involves skin tightening, fat reduction, volume restoration, or tissue repositioning—and whether the expected degree of change is realistic.
Potential risks and side effects differ by procedure:
- Ultherapy PRIME, Thermage FLX, and Sofwave: Treatment may cause heat-related discomfort, redness, swelling, tenderness, bruising, or altered sensation such as numbness and tingling. Burns and nerve-related complications are also possible with energy-based procedures.
- Hyaluronic acid fillers: Swelling, pain, bruising, infection, and lumps can occur. Accidental injection into a blood vessel can cause tissue damage and, in rare severe cases, blindness or stroke.
- FatX Core injections: Pain, swelling, redness, bruising, and inflammation can occur after injection. The degree of swelling and response to treatment differ between patients.
- Surgical lifting procedures: Incisions, scars, anesthesia-related complications, bleeding, infection, altered sensation, and a longer recovery period need to be considered.
Tell your physician if you are concerned about facial hollowing or have previously lost facial volume after an aesthetic procedure. Tightening treatment alone may not address a face that already lacks fullness.
Planning Recovery and Follow-Up
Recovery needs depend on the treatment area, technique, medical history, and individual response. Even when a procedure does not require an incision, redness, swelling, tenderness, or other complications can affect your plans.
Before treatment, tell us about recent cosmetic procedures, including those performed outside Japan. Please tell us if you have received a skin or laser treatment within the past month, HIFU or RF treatment within the past three months, or have had significant sun exposure or sunburn within the past month. We may be unable to perform treatment in these circumstances. Our physician will also assess your current skin condition.
Ask when you can resume your usual skincare, makeup, exercise, and other activities. Follow the instructions for your specific treatment rather than assuming that every non-surgical procedure has the same recovery requirements.
If you are visiting Tokyo, share your travel itinerary during the consultation so that treatment and follow-up needs can be discussed.
If you have questions or concerns after returning home, you can contact MATSURIKA CLINIC GINZA by email or LINE. If you develop severe pain, sudden swelling, bleeding, fever, visual symptoms, or signs of an allergic reaction, seek urgent medical care where you are rather than waiting for an online response.
Questions to Ask at Your Jawline Consultation
These questions can help you turn a general concern about sagging into a specific treatment discussion. The answers depend on what your physician finds during the examination.
1. How Do I Know Which Jawline Treatment to Discuss?
Start by identifying whether your main concern is loose skin, fullness under the chin, jowls, or loss of facial volume. You do not need to choose a device before the consultation. At MATSURIKA CLINIC GINZA, our physicians assess the broader facial contour and explain which available treatments address the contributing tissues.
2. Is Ultrasound or Radiofrequency More Appropriate for a Sagging Jawline?
Neither is automatically the right choice for every sagging jawline. Ultherapy PRIME uses focused ultrasound, Sofwave uses SUPERB ultrasound, and Thermage FLX uses monopolar RF; their heating patterns and main target tissues differ. The decision depends on your skin condition, treatment goal, previous procedures, and the area that needs attention.
3. Are Jawline Treatments Painful, and What Are the Main Risks?
Jawline treatments can cause discomfort, and the type of pain and risk depends on the procedure. Energy-based treatments may produce heat or tenderness, while injections may cause stinging, swelling, or bruising; more serious complications are possible with either category. At our clinic, topical anesthesia costs ¥3,300 and a nerve block costs ¥5,000, both tax included, when applicable.
4. How Much Does Jawline Treatment Cost in Tokyo?
The cost depends on the treatment method, area, provider, and amount of treatment. For example, we charge ¥277,820 for Ultherapy PRIME with 348 shots, ¥327,800 for physician-performed Thermage FLX with 300 shots, and ¥308,000 for Sofwave covering the full face and under the chin, all tax included. These are different treatment plans, so confirm the full estimate, including extra fees, before booking.
5. What If Non-Surgical Treatment Does Not Match My Goals?
Non-surgical treatments may not provide enough tissue repositioning for pronounced jowls or substantial excess skin. Your physician can explain the limits of the clinic’s available treatments rather than treating additional sessions as a substitute for surgery. If a surgical approach better matches your goals, seek an assessment from a qualified surgeon and compare the recovery requirements and risks.
Summary: Choosing Jawline Treatment by Cause
Choosing jawline treatment starts with understanding whether loose skin, under-chin fullness, or changes in facial volume are affecting your lower face. During your consultation, we assess these factors and discuss the treatment options and their limits.
