Skin Tightening vs. Skin Lifting: Which Concern Should You Discuss in Tokyo?
“Skin tightening” and “skin lifting” are often used as if they mean the same thing, but they describe different changes. Tightening usually refers to firmer skin with a smoother texture. Lifting usually refers to a change in facial contour, such as a more defined jawline or less heaviness in the lower cheeks.
Knowing which change you want helps your physician choose which treatments to discuss. Some energy-based treatments mainly work on the skin, while others reach deeper tissues that support facial contour. Some concerns, such as lost volume or significant loose skin, may need a different approach altogether.
This guide explains how skin tightening differs from skin lifting, how to describe your concern at a consultation in Tokyo, and how we at MATSURIKA CLINIC GINZA connect your goals with treatment options, expected changes and limitations.
What Do Skin Tightening and Skin Lifting Mean?
Skin tightening focuses on the quality of the skin, while skin lifting focuses on the shape and position of facial tissues. Many people notice both changes at the same time, but separating them makes treatment planning clearer.
Firmness and Skin Texture
Tightening concerns are about the skin itself. You may notice that your skin feels less firm, looks crepey or has fine lines that were not there before. These changes are linked to the dermis, the layer of skin that contains collagen and elastic fibers.
Treatments discussed for tightening usually heat the dermis to stimulate collagen remodeling. The goal is firmer, smoother-looking skin rather than a major change in facial shape.
Contour and the Position of Facial Tissues
Lifting concerns are about contour. You may notice jowls near the corners of the mouth, a softer jawline or heaviness in the lower cheeks. These changes can involve deeper layers, including fat and the superficial musculoaponeurotic system (SMAS), a fibrous layer that supports facial tissues.
Treatments discussed for lifting usually reach deeper tissues. Even so, non-surgical treatments produce more limited changes than surgical lifting, and the degree of change varies from patient to patient.
| Concern | What you may notice | Main tissues involved |
|---|---|---|
| Skin tightening | Less firmness, crepey texture, fine lines | Dermis |
| Skin lifting | Jowls, a softer jawline, heavier lower cheeks | Fat layer and SMAS, along with the skin |
| Both | Loose skin together with contour changes | Several layers at once |
Which Change Are You Hoping to See?
Before your consultation, try to describe the change you want rather than the name of a treatment. A clear description helps your physician judge whether tightening, lifting or another approach matches your concern.
Describing Loose Skin and a Less Defined Jawline
Specific words help. Instead of saying “I want lifting,” describe where you see the change and what bothers you about it. For example:
- “The skin on my cheeks feels thinner and less firm.”
- “My jawline looks less defined, especially from the side.”
- “I notice heaviness near the corners of my mouth.”
- “The area under my chin looks fuller than before.”
It also helps to tell us how much change you hope for, when you first noticed the concern, and whether it bothers you most in photos, in the mirror or in certain lighting.
Using Photos to Explain Your Goals
Photos of yourself from a few years ago can show how your contour has changed. If you bring them, choose images with a similar angle and lighting. Photos of other people can show the general direction you like, but they cannot predict your result because facial structure and skin differ from person to person.
How Doctors Connect Treatment Goals With Options
After examining your skin and contour, your physician matches your main concern with the tissues that need to change. At MATSURIKA CLINIC GINZA, we consider several options rather than recommending a single device to everyone.
Discussing Energy-Based Treatments
We offer several energy-based treatments for sagging skin. They differ in energy source, main target tissue and treatment purpose.
| Treatment | Energy source | Main target | Main purpose to discuss |
|---|---|---|---|
| Ultherapy PRIME | Focused ultrasound (HIFU) with real-time ultrasound imaging | Deeper tissues, including the SMAS, at selected depths | Lifting-oriented concerns such as jawline and lower-face contour |
| Thermage FLX | Monopolar radiofrequency (RF) | Dermis and the tissue beneath it | Overall firmness and skin tightening |
| Sofwave | SUPERB™ ultrasound (parallel beams) | Dermis, centered at about 1.5 mm | Skin firmness, fine lines and wrinkles |
| ULTRAcel Zi | Focused ultrasound (HIFU) with dot and linear modes | Different layers depending on the mode | Sagging and localized fullness, depending on the mode |
The table shows differences in approach, not a ranking. The same device may be part of a plan for both tightening and lifting, depending on the area and settings. For Ultherapy PRIME, our physicians consider where to treat, which depths to target and how to distribute the shots, rather than relying on shot count alone.
Regulatory information: Ultherapy PRIME and Thermage FLX are FDA-cleared in the United States for their respective cleared uses. FDA clearance does not mean that every treatment purpose has been cleared, and it does not constitute approval in Japan. The Ultherapy PRIME, Thermage FLX and ULTRAcel Zi devices used at our clinic have not been approved under Japan’s Pharmaceuticals and Medical Devices Act. Ultherapy PRIME is obtained through Cynosure Japan / Syneron Candela K.K., Thermage FLX through JMEC Co., Ltd., and ULTRAcel Zi through Jeisys Medical Japan, Inc. ULTRAcel Zi is approved by the Ministry of Food and Drug Safety in South Korea. Sofwave is approved in Japan for improving moderate to severe wrinkles of the face and neck by heating the dermis; this approval does not cover every lifting purpose or guarantee results.
When a Different Treatment Approach May Be Needed
Energy-based treatments do not address every cause of sagging. Your physician may discuss a different approach when:
- Facial volume has decreased: hollowness in the cheeks or temples may relate to volume loss, and hyaluronic acid fillers may be discussed.
- Localized fat is the main concern: fullness under the chin may relate to fat rather than loose skin, and fat-reduction options such as FatX Core may be discussed.
- Significant tissue descent or excess skin is present: non-surgical treatment may not produce the change you want, and a surgical consultation may be more appropriate.
Comparing Expected Changes and Limitations
When comparing options, look at what each treatment can realistically change, how long recovery takes, what risks apply and how much the complete plan costs.
Why Treatment Names Alone Are Not Enough
Two plans with the same treatment name can differ in area, depth, energy settings, shot count and provider. A plan for “HIFU” may also involve different devices. Ask your physician which tissue each part of the plan is meant to address and which concern it is unlikely to change.
Changes after energy-based treatment usually develop gradually as collagen remodels, and the degree of improvement varies from patient to patient. One session may not address both firmness and contour to the extent you hope.
Recovery, Risks and Total Cost
Energy-based treatments can cause heat-related discomfort during treatment. Potential risks and side effects include redness, swelling, tenderness, mild bruising and temporary changes in sensation, such as numbness or tingling. Burns and nerve-related complications are also possible. Recovery needs depend on the treatment, area and settings, so ask what to avoid afterward and for how long.
Below are examples of our fees. All prices are in Japanese yen and include tax.
| Treatment | Area and quantity | Provider | Price |
|---|---|---|---|
| Ultherapy PRIME | Full face (including under the chin), 464 shots | Physician | ¥347,270 |
| Thermage FLX | Full face and under the chin, 300 shots | Physician | ¥327,800 |
| Thermage FLX | Full face and under the chin, 300 shots | Nurse | ¥272,800 |
| Sofwave | Full face + under the chin | Physician | ¥308,000 |
| ULTRAcel Zi | Dot mode, full face (including around the eyes), 500 shots | Ask at consultation | ¥100,000 |
These are price examples, not equivalent plans, because the area, depth and shot count differ. For nurse-performed plans, a physician examines you first and decides the treatment area, and a nurse then performs the treatment. Consultation and examination without treatment cost ¥11,000 (tax included). Anesthesia options other than the topical cream included with Sofwave are charged separately, and some fees differ for appointments that require language support. Cosmetic treatment at our clinic is self-pay and is not covered by Japanese public health insurance. Please review our English price list and confirm the full estimate before treatment.
Questions About Tightening and Lifting
These questions can help you prepare for your consultation.
1. How does skin tightening differ from skin lifting?
Skin tightening aims to improve firmness and texture in the skin, mainly in the dermis. Skin lifting aims to improve contour, which can involve deeper tissues such as fat and the SMAS. Many people have both concerns, so your physician examines each area before suggesting a plan.
2. Can one treatment address both concerns?
Sometimes, but not always. Depending on the depths and areas treated, one energy-based treatment can affect both firmness and contour to some degree. If your concerns involve several layers, your physician may suggest combining treatments or explain which concern will remain.
3. How should I explain my main concern at a consultation?
Describe where you see the change, what bothers you about it and how much change you hope for. Words such as “less firm,” “crepey,” “heavier jawline” or “fullness under the chin” are more helpful than a treatment name. Photos of yourself from the past can also help.
4. Ultrasound or radiofrequency: which suits lifting concerns?
Neither is better in every case. Focused ultrasound such as Ultherapy PRIME can reach deeper tissues at selected depths, while monopolar RF such as Thermage FLX heats the dermis and the tissue beneath it more broadly. The appropriate choice depends on your skin, contour, treatment history and goals.
5. What if my concern needs more than a non-surgical treatment?
If your main concern is significant tissue descent or excess skin, non-surgical treatment may not produce the change you want. In that case, we explain the limits of non-surgical options and the reasons another approach, including a surgical consultation, may better match your goal.
Summary: Choosing Your Skin Tightening or Lifting Goal
Skin tightening focuses on firmness and texture, while skin lifting focuses on contour and the position of facial tissues. Identifying which change matters most to you, and where, helps your physician decide which treatments to discuss and what each one is unlikely to change.
At MATSURIKA CLINIC GINZA, we examine your skin and contour, then explain options such as Ultherapy PRIME, Thermage FLX, Sofwave and ULTRAcel Zi, as well as approaches for volume loss or localized fat. We also explain when non-surgical treatment may not meet your goal.
We have English-speaking physicians and staff, and we can use translation tools to support communication. Patients who do not speak English or Japanese are encouraged to attend with a professional medical interpreter, who should bring documentation of their credentials. If you need language support, please book through the dedicated option for non-Japanese speakers. We allocate additional consultation time for these appointments. If you have questions or concerns after returning home, you can contact MATSURIKA CLINIC GINZA by email or LINE. For symptoms that may need urgent care, such as severe pain, sudden swelling, bleeding, fever or signs of an allergic reaction, please seek medical care promptly at a local healthcare facility.
To discuss whether tightening, lifting or both match your concern, book a consultation with MATSURIKA CLINIC GINZA.
